Many individuals live their lives with chronic diseases that means regular contacts with the health care system and a need for successful selfcare. Being knowledgeable about one's disease is beneficial both for the individual and the wider community.

This project targets individuals with e. g. cardiovascular disease, chronic pulmonary disease or type II diabetes. The aim is to enable patients to monitor symptoms and signs from their disease, learn more about selfcare and lifestyle behaviour change.

An intervention will be conducted for each disease group. Tailoring will be made by panel evaluations, usability tests and focus groups with patients and healthcare professionals. Later, the intervention will be evaluated in a randomized controlled trial.
