Coordinated Dementia Care for Older People Living at Home

As the population ages, dementia is becoming one of the biggest challenges for health and social care systems. Care for older adults living at home is often fragmented. Different organizations are responsible for different parts of care, which can make coordination difficult.

In Sweden, the aging in place policy means that most people with dementia remain in their own homes, supported by both municipal and regional services. While this is often beneficial for individuals, it also places high demands on the care system.

DEM(H)CARE is an interdisciplinary research programme aiming to improve coordination between home care, municipal health care, and primary care for people with dementia living at home. The programme addresses the urgent need to develop best practices for organizing and delivering care in a more cohesive, person-centred way.

Research Team

The DEM(H)CARE team consists of researchers from various disciplines with strong experience in working together on complex issues in elder care and health care.

Their combined expertise ensures that the programme is grounded in both scientific rigour and practical relevance. To support future research in the field, the programme actively involves junior researchers and offers opportunities for skill development and international networking.

Through seminars, workshops, and conferences with leading experts, DEM(H)CARE also promotes long-term capacity building in dementia care research.
